Ted wanted a visual summary of his three outcomes (to get a flat, a job and a car) so that he could have a reminder of this.
The advocate had gathered person-centred information about what was working and not working for Ted, his career ambitions and his perfect week. This was recorded and jointly held, along with the outcomes and actions.
This information was also useful for the commissioners to inform the Working Together for Change process.
