At the end of the conversation, the discussion and key outcomes are recorded in Kathy's care and support plan, including the things Kathy intends to do and what named others will do. This includes the medication that she is taking for her diabetes and what to do if she is worried about her health.

Kathy wants to share this with occupational health at work and thinks she will find her 'background story' helpful to share with colleagues and at the dementia café as well. This information is uploaded to Kathy's electronic health record, together with the care and support plan and Kathy keeps her own paper copy.

She also agrees that information from the planning process can be used anonymously and shared to inform commissioning (for example see Working Together for Change) and can be shared with the Alzheimer's Society as well. This is to help inform what needs to change to create a more dementia friendly community.
