What our organisation does to support people with care and support needs:
Our skilled Support Workers provide social care support for over 1,500 customers in the community, in the home and through short break respite support for families.
We support individuals with learning disabilities physical complex needs, autism, ADHD or an acquired brain injury. We can help if you need support to remain independent through age or ill health, dementia or are a young person making the transition to adult services.
Our aim is to support you to lead the life you choose.

What we will do to Make it Real
Develop Learning Programmes within the Home
Identify Health Initiatives and Information to promote wellbeing in the home
Develop an Easy Read Support Plan for Tenants
Explore local advocacy, befriending and volunteering networks.

I am supported by people who see me as a unique person with strengths, abilities and aspirations
This will link to updated support plans, a skills development drive and promotion of progression, goal setting and person centred reviews to chart and log outcomes.
How we will co-produce and with whom
We will develop and utilise new tools with our customers in our Supported Housing and Supported Living Schemes.
We will work with other local charities, organisations and groups to raise awareness and access further opportunities.
