TLAP response to the Universal Personalised Care – Implementing the Comprehensive Model

"I applaud the aspiration of the Universal Personalised Care Comprehensive Model. It demonstrates the NHS’s shift to a more personalised approach to health where people with long-term and mental health conditions can rightfully expect support which fits around the context of their lives, rather than patients to be solely diagnosed and treated.
 
"From TLAP’s long experience of personalisation in social care we endorse the view that people are the best integrators of their own care. The approach set out in the Comprehensive Model presents a real opportunity for meaningful conversations with citizens to help them exercise choice and control. But we must not underestimate the enormity of the cultural shift that is required across the workforce to embed this practice so that people’s experiences of care and support is personalised on each and every occasion. 
 
"Making it Real sets out a framework for what good personalised care and support looks like and we believe it can play a significant role in supporting the ambition and implementation of the Comprehensive Model. 
 
"The commitment to put health inequalities at the core of the personalised care agenda is welcomed and essential. TLAP will pro-actively support taking forward the work in this area. In my personal capacity, as a member of the NHS England Health Inequality Board, I will continue to highlight the imperative of taking action to close the gap in the health inequalities that currently exist for people from BAME groups".
