It is vital that people with long-term conditions have the opportunity to have a conversation about what matters to them and to be seen as an equal partner in their care by those supporting them, in health and social care.

This tool has the potential to ingrain this best practice across the entire health and social care sector, providing organisations with a baseline starting point and the ability to continually assess their progress.

This self-assessment tool is targeted at teams who provide care for individuals who have complex care needs. 

This resource has been co-produced with the Coalition for Collaborative Care, Think Local Act Personal, Health Education England, the Royal College of GPs and Progress for Providers series the Integrated Personal Commissioning team, with their Voluntary sector partners Helen Sanderson Associates and the Alzheimer’s Society.
