The 'People Matter' strategy developed by the JIP contains key objectives aimed at supporting people to live their own lives and choose services which are high quality, safe and promote independence, wellbeing and dignity. The central planks of the 'People Matter' strategy are personalisation, commissioning, improving performance and efficiency.
Below, we introduce the full range of our personalisation projects which taken together provide a framework for supporting implementation and will drive forward commissioning and personalisation across the region.
Assessment and Resource Allocation
Improving Best Practice in Yorkshire and Humber in relation to resource allocation, self directed support and the personalised assessment process.

Care Management and Social Work
The project includes four key work-streams which together support the cultural shift towards a self-directed support approach for care management and social work within the region.

Community Capacity
This project aims to share learning from existing good practice in relation to community capacity building across the region and to develop a sustainable learning network. A web based regional network will provide mutual learning and support for community capacity building

eMarketplace
This project is focussed on the development and delivery of an eMarketplace across the region that supports market development, market management, and access to information. It will also deliver key benefits to users such as more responsive services, greater control over the procurement type processes that affect them, and more informed choice.

Health
Work is being developed with health partners to co-produce a framework for individualised support and personal health budgets. This work is being developed in line with approaches in social care in order to maintain continuity for people on the receiving end and to provide individuals with greater choice and control.

New Forms of Support
The objectives of this project are to identify the conditions which enable communities to create, support and develop commissioning of micro-services and to work with local people to understand how small providers can best meet unmet needs.

Outcomes and Quality
This project will co-produce an outcomes framework to form the basis of delivering individualised support across social care and other areas of the personalisation agenda.

Race and Social inclusion
This project is now completed. It worked to increase involvement and engagement in services from BME and socially excluded individuals and communities, leading to citizens feeling empowered and able to direct their own services.

Reablement
This project aims to identify best practice across the region in relation to the delivery of reablement services and to ensure that this is disseminated to other councils to reduce the duplication of effort and systems.

Service Redesign ( internal )
This project is designed to demonstrate the development and redesign of internal day services in response to the personalisation agenda - focusing on both staff and service development, offering eligible service users the opportunity to explore personal budgets and ensuring all service users have personalised support plans which link to outcomes.

Support Brokerage
Within personalisation, an effective support system is crucial for those people directing their own support. The overall aim of this project is to develop a common framework for support brokerage across the region that will support planning, brokerage and management of budgets.
