Carers' Care Co-ordination Pilot

This report is based on a pilot project that looks at changing the way we assess substance misusers at the point of presentation to treatment services. The project seeks to identify hidden adult carers (including kinship carers) and route them into specialist support services and statutory carers' assessments where indicated. As part of a care co-ordinated approach to facilitating a client's journey through a treatment episode, carers are supported to access their own psychological, emotional, practical, financial and peer support. The project helps carers to understand the treatment journey (through different modalities and providers) and how to support their loved one's recovery, whilst avoiding harmful relationship dynamics.
