Transformation of Adult Social Care

Transformation of adult social care is a system wide transformation of adult social care aimed at radically changing the way in which people are provided with care and support. Change has been implemented across the four domains of building community capacity (social capital), promoting access to universal services, developing prevention and early intervention, and introducing self-directed care to increase control and choice through the introduction of Personal Budgets. A substantial programme of work was undertaken through the South East Joint Improvement Partnership to assist local authorities with the transformation.
