Case Studies from the Dementia Programme in the East Midlands

The East Midlands Dementia Programme has been a collaborative project between the Directorate of Public Health and Social Care, NHS East Midlands and the East Midlands Improvement and Efficiency Partnership, working together with people living with dementia, their family carers, and health and social care communities. The case studies below highlight the progress that has been made across the region.
