Consultation on Personalised care and support planning tool.

We are seeking your views on TLAP's latest online resource to support Care Act implementation.

The Personalised care and support planning tool is designed to support work to deliver person-centred, coordinated care across the health and social care system.

It is aimed at leaders, commissioners, planners, clinicians and people who use services engaged with the practical and workforce challenges involved.

There are case study examples of characters with physical, mental and social care issues who are eligible for personal budgets for health and/or social care. The tool shows how their interactions with different services, clinicians and professionals means they experience a joined-up approach with improved health and wellbeing outcomes.

Feedback so far has been helpful and positive. We've had responses from nurses, social workers, carers and commissioners.
