Examples of where Making it Real is Making a Difference Call for Case Studies

Added on

As a result of adopting Making it Real has your organisation seen real change which has helped people lead better lives? If so, then TLAP would very much like to hear from you.

What is Making it Real?

Making it Real sets out what people who use services and carers should expect if their care and support is truly personalised and integrated.

Since its inception in 2011 over 900 organisations have signed up to using Making it Real and its value and relevance has been highlighted as good practice in the Care Act Guidance.

Building on this strong success we want to make sure that the experience and learning of using Making it Real is shared widely and promoted. We are therefore looking for good practice examples that show how organisations have improved their approaches to the provision of care and support in one or more of the Making it Real themes:

  • Information and advice: having the information I need when I need it
  • Active and supportive communities: keeping my friends, family and place
  • Flexible integrated care and support: my support, my own way
  • Workforce: People who support me
  • Risk enablement: feeling in control and safe
  • Personal budgets and self-funding: my money

Can you help?

We are particularly interested in hearing examples of how Making it Real has encouraged creative and innovative approaches to supporting and working with older people and adults with mental health needs.

Achieving personalisation with these two groups can be viewed as more difficult. We’d like you to tell us how and what your organisation has started to do differently and the impact that this is having on the people you support.

If your experience is with other groups, let us know and we will still consider your cases.
