Delivering Care & Support Planning has been developed with people who use services, families and carers to show what good care and support planning looks like in practice.

It is backed up with examples from councils across England that are leading the way in this area. The guide describes what people want in a care and support planning process and the elements that need to be in place to make this happen. There are also a set of recommendations for councils so that they can be both Care Act compliant and person-centred in their approach.

Delivering Care & Support Planning offers councils ideas, lessons and practical examples as well as template care and support plans.

It is part of a suite of resources commissioned by the Department of Health in partnership with the Local Government Association and the Association of Directors of Adult Social Services to support local government in implementing the Care Act 2014.
