    This project supports the regional implementation of Objective 6 of the National Dementia StrategyThe project will seek to enhance the implementation of Objective 6 through the following high-level outcomes:Development of the market to ensure there is a range of services for people with dementia living at home and their carers that meet the needs, choices and aspirations of those who use them. Supporting existing providers to develop high quality services for people with Dementia Reducing the culture of risk aversion when working with people with dementia and reducing/removing stigma as a barrier to equality The project has the following objectives:To explore the experience, needs and desires of people with dementia and their carers based on a programme of engagement and a literature review of existing regional and national evidence.

  • Improving Public Relation and Communication in Personalisation across the North East

    This project aims to support local authorities to develop and enhance skills necessary to support effective communication regarding the transformation of Adult Social Care. The main input and facilitation for the project will come from communication and PR expertise.

  • Exploring the Electronic Market for Social Care in the North East

    This project assessed the viability of the electronic social market place for the North East. The project undertook an appraisal of the present state of the local market in two specific areas of the region.

  • Innovative Practice for PPF in the West Midlands

    Innovative Practice - case examples from the West MidlandsAs part of the National Delivery Support Plan, Jeff Jerome recently requested examples of innovative and interesting practice across all the milestone areas that could usefully be shared. He will be interrogating the data from the PPF Survey conducted earlier this year as one means of identifying potential examples of innovative practice and also asked regional transformation leads to let him know about potential innovative practice in our region.

  • Local Area Coordination in the North East

    This project is exploring how the Local Area Coordination approach can support the development of Personalisation and self directed support. Local Area Coordination (LAC), developed in Australia in 1988, offers a way to build individual, family and community self sufficiency so that disabled and older people can choose to live with their families, or in their local community without compromising their quality of life.

  • North East Autism Consortium

    The North East Autism consortium (NEAC) will drive forward the modern commissioning of care and support services for people with Autistic Spectrum Disorders and Asperger syndrome. This will be achieved in two ways:Via the delivery of the actions plans of the 4 sub regional Autism Service Development Groups (ASDG) developed from stakeholder events in 2008, which will lead to the creation of modern, cost effective and individualised services for people living in our region.

  • Supporting People Cost Benefit Analysis

    An in-depth analysis of the benefits of the SP programme to highlight learning for Personalisation in the North East. This project will undertake an in-depth regional analysis of the benefits of the Supporting People Programme to highlight where efficiencies and good commissioning practise could affect the costs of procurement of Social Care Services for Independent Living, in advance of the mainstreaming of the supporting people programme by March 2011.

  • Supporting Social Care Change Management in the North East

    This project sought to support the transfromational change necessary in Adult Social Care and to provide community and council leaders with relvent change management tools. This project will support the transformational change within Adult Social Care and provide individual Council and community leaders with significant change management information, theory, practical tools and an action focused process for delivering specified change in their local area.
