The two Hertfordshire PCTs have put their carer's breaks money into a section 75 agreement and Hertfordshire Adult Care Services act as lead commissioner to ensure that new monies are spent in a way that is fully integrated with the Carers Grant spending. A new carers breaks and information service (Making a Difference) helps refer carers for information or support.

Book Your Own Breaks is a partnership between Hertfordshire County Council, Slivers of Time, Crossroads Care Hertfordshire and Carers in Hertfordshire. The project enables carers to book care workers on line at anytime, and to get the particular support worker they want at the time and location of their choosing without having to deal directly with payment and administration.

Carers in Hertfordshire and Hertsmere NHS PCT and GP surgeries have worked together to develop enhanced services in GP surgeries to identify and support carers. A survey was carried out in 2005 to look at what difference this had made and what the benefits to carers were.

The Needs and Outcomes Assessment developed by Hertfordshire takes into consideration the views and potential needs of the carer throughout the assessment process for the person who is cared. By considering the carer under each domain the aim is to understand both what a carer is able but also importantly willing to do.
