Hertfordshire Needs and Outcomes Assessment form

The Needs and Outcomes Assessment developed by Hertfordshire takes into consideration the views and potential needs of the carer throughout the assessment process for the person who is cared. By considering the carer under each domain the aim is to understand both what a carer is able but also importantly willing to do. The outcomes of this should then be taken into consideration when undertaking a carer's assessment.
